Auto-enrollment and opt-out form

Withdrawal request

Use this form to request to withdraw money from your New York Secure Choice account.

Removal of excess contributions

Use this form if you need to remove excess contributions because your contributions exceed the annual Roth IRA contribution limit, or your income exceeds the maximum allowed for Roth IRA contributions.
